Medical Staff Application Request Process

1. All applicants for medical staff membership and clinical privileges must complete the Application Request Form.You may obtain a copy of this form by calling the Medical Staff Services Department at 443-777-7410, emailing Brenda.Boblitz@medstar.net or by downloading the form from the website. To expedite the process, please fax or email the completed form along with your current curriculum vitae to the Medical Staff Services Department. Our fax number is 443-777-7558. Please note, however, that the form may also be mailed to the following address:

2. Upon receipt of the completed Application Request Form, the Medical Staff Services Department will review it and then forward the necessary documents to the applicant via email or by mail.

AHP Initial Application Process for NPs, CRNAs, CNMs, PAs, Psychologists, LCSW-C, LCPCs, and Audiologists:

1. The above noted practitioners must have accepted a position at MedStar Franklin Square Medical Center or be employed by a physician on our medical staff. Please do not request an application unless this has taken place.


2. All applicants for clinical privileges must complete the Maryland Hospital Credentialing Application. You may obtain a copy of this application by calling the Medical Staff Services Department at 443-777-7410 or by emailing Brenda.Boblitz@medstar.net.
